Writing this book has brought me a peculiar mixture of both anguish and joy. The message is basic. While acne is a complicated, mysterious and devious disease, the treatment is remarkably simple and control is finally possible. That is the joy. The anguish lies in the personal memories I dislodged as I wrote. I traveled long journeys to come to the end of my acne. The medical establishment miserably failed me in my search for a treatment that would work. It is out of those failures that this book rises.

My severe cystic acne, which began in childhood and continued unabated into my twenties, was the impetus which led me into a career as a dermatologist and biochemist specializing in the research and treatment of acne. My professional obsession has been to discover methods which though too late for me personally, would save others from the scars of acne. Good Luck with your program.

About the Author
Dr. Fulton graduated from Tulane University Medical School in 1965, received a Ph.D. in Biochemistry from the University of Miami in 1972 and is a fellow of both the American Academy of Cosmetic Surgery and American Academy of Dermatology. He is board certified in both specialties.

He is constantly on the leading edge of new breakthroughs in product development. He developed tretinion (Retin A®) with Dr. Albert Kligman for the University of Pennsylvania in 1969, benzoyl peroxide gel (Panoxyl®) and topical erthromycin(E-Gel®) with Dr. Harvey Blank for the University of Miami in 1975. His latest advancement in skin care therapeutics is the topical use of vitamin A propionate -US patent 5,043,356.

Dr. Fulton's achievements extend into the area of corrective chemical peels and facial laser abrasion for skin rejuvenation and the improvement of acne scars. He is a pioneer in dermatological cosmetic surgery techniques and has published more than 100 medical articles.

Acne Rx
This is a comprehensive book by the co-developer of Retin A, James E. Fulton Jr. MD, Ph.D. The book discusses the anatomy of skin and how it relates to the acne process. The author demonstrates that acne is a genetic process. The cells of the follicles of skin sludge up and stick instead of migrating out with the oil in certain families. This sticking of the dead skin cells increases until they are visible as small milia or closed and open comedones. In some individuals these lesions become quite inflammatory developing the papules, pustules, and cysts. Once this basic understanding is clear, it’s obvious that the myths of dirty skin, dietary restrictions, and sexual frustrations are not significantly important in the development and progression of the disease.

He discusses external aggravating factors such as the climate, humidity, and cosmetics. Then, he switches to the internal aggravating factors such as iodides, hormone imbalances and stress.

The second half of the book Acne Rx is dedicated to old pore and new acne treatments. He discusses the extensive use of tetracyclines for acne suffers and that these do not have a clear benefit and are not indicated in many cases. He discusses the newer topical treatments, which evolved around Vitamin A, such as Vitamin A Acid, and the esters of Vitamin A such as Vitamin A Propionate and Vitamin A palmitate. There is also discussion of Benzoyl Peroxide Gels and Lotions. These penetrate into the pores, release free radical oxygen, kill the bacteria, and irritate the pores lining so cells are produced at a rapid rate that do not stick. The acne problem is improved. He discussed at length the use of Accutane, which is a isomer of Vitamin A acid. When taken orally the isomer decreases the sebum production, and the bacteria count of the skin. This results in an improvement in acne, which has the exciting consequence of giving a permanent cure in some cases. In summary, he suggests a topical treatment with Vitamin A and Benzoyl Peroxide Gels and Lotions followed up with the use of Accutane, if necessary.

Following this discussion of skin therapy, the author reviews the consequence of emotional distress and develops a therapeutic program to improve the outlook of the individual after the acne is improved. There is also a section on the physical scars and their improvement with dermabrasion. Finally, there is an epilog section where he gives the reader the opportunity to fill out an acne history questionnaire and send it to the author so a wide database of information can be developed. Over all, the book is 300 pages of in-depth information on acne for the serious acne sufferer or for the physician to familiarize himself with the causes and treatment of acne.
